NEW PATIENTS TO THE CLINIC
Welcome to MISH hospital and clinics. We welcome all patients, including patients from other weight loss surgery clinics, and patients that already underwent weight loss surgery. Below please use the “Registration” menu to complete your registration with our clinic. Complete both the registration and sleep apnea screening forms. The forms should not take more than 5 minutes to complete. Please also check out “A word about home medications” to better prepare for your first office visit. Please be prepared for your first visit to take about 1 hour.
PREPARING FOR WEIGHT LOSS SURGERY
There are several things you can start doing to get ready for your weight loss operation while completing your medical work-up and meeting your insurance requirements. Educate yourself about obesity and weight loss surgery by exploring our website. Learn about the risks associated with obesity, about your weight loss surgery options, and the risks and benefits of bariatric surgery. We encourage all patients to quit smoking before surgery – please access the great resources offered by the state below. Start discussing treatment options with your primary provider to help quit smoking. Use the surgery preparatory information menu below which includes a checklist and a Guide to prepare and educate yourself about surgery in general, and start reading the diet guide to prepare for your weight loss operation.

Follow-Up
Follow-Up is a very important for long term success. Please refer to the follow-up schedule for details on when you should be seen next. Refer to the lab schedule for details on what labs and when to have them drawn. Please complete the Progress Follow-up questionnaire prior to your follow-up office visit, by clicking on the link.
RULES CLASS SUPPORT GROUP
The rules class–support group is KEY to long-term success. We want weight loss surgery to be a long-term solution to your weight loss goals. Part of your success will depend on how well you are educated about your 'tool'. Hunger and portion control is achievable. The goal of the class is to teach you how to use your operation effectively while changing your eating habits and lifestyle.
