Request your medical records
Patients and authorised representatives can request records from MISH Hospital and our clinics through a secure online form. Read what you'll need first — incomplete requests are the main reason records take longer than they should.
What you'll need
You'll upload documents as part of the form, so have them photographed or scanned before you begin. Each image must be under 10MB.
If you're the patient
One document, and your own details.
- A photo of your driver's licence or other government-issued ID
- Your date of birth, address, phone and email
- The approximate dates of the care you need records for
If you're requesting for someone else
Two documents: your own ID, plus proof of your authority.
- A photo of your own government-issued ID
- One of the supporting documents listed below
- The patient's name, date of birth and contact details
Accepted proof of authority
Any one of these, provided it covers the release of medical information:
- Power of Attorney for Health Care
- General Power of Attorney, where it includes medical decision-making or release of records
- Advance Healthcare Directive, in effect at the time of the request
- Letters Testamentary or court appointment as executor, for a deceased patient
- Death certificate, for a deceased patient
- Other documentation, subject to review

What you'll be asked
Six sections, about ten minutes. Nothing is saved between visits, so set aside the time.
Are you the patient?
Answer Yes if the records are your own, or No if you're a legal or personal representative. Answering No adds the supporting-documentation upload.
Patient information
The patient's full name, date of birth, address and phone number. An email address is required so we can reach you about the request.
Which records, and from where
Choose MISH Hospital, MISH Clinic, or both. If you're requesting clinic records, select the physician you saw from the list.
Where to send them
Select To Patient to have records sent to the address you've already entered. To send them elsewhere — another physician, an attorney, an insurer — choose Other and give that recipient's name, address, phone and fax.
Reason, dates and record types
Pick one reason, give the approximate date range of care, and select the record types you need: visit notes, operative reports, lab results, pathology, radiology, sleep studies or hospital stay records. If none of those fit, describe what you're looking for in your own words — the more specific you are, the more completely we can respond.
Signature and date
A signature and date are required for the authorisation to be valid. If you're signing on the patient's behalf, also print your name and your relationship to them.

What happens after you submit
Timing
Requests are fulfilled within 30 days. If yours is urgent — a surgery date, a specialist appointment — call Medical Records and tell us, and we'll prioritise it.
Fees
Requests of 10 pages or fewer are free. Larger requests may carry a fee. We'll tell you the amount before we process anything, so nothing arrives as a surprise.
Your rights
Signing is voluntary and refusing won't affect your treatment or your coverage. You can withdraw the authorisation in writing at any time, and you're entitled to a copy of what you signed.
Common questions
I need my records for an appointment next week.
Submit the form, then call Medical Records at 913-322-7408 and say the request is time-sensitive. We can often move quickly when we know there's a date attached — but we need to know.
Can my records go straight to another doctor?
Yes. In the "Where do you want the records sent to?" section, choose Other and enter the receiving practice's name, address, phone and fax. Sending records directly is usually faster than routing them through you.
I'm requesting records for a relative who has died.
You'll need your own photo ID plus proof of your authority over the estate — Letters Testamentary or a court appointment as executor. A death certificate alone may not be enough on its own. Call 913-322-7408 if you're unsure what you have.
I don't know which doctor or which dates.
Give your best estimate and describe what you're looking for in the free-text field — a condition, a surgery, a hospital stay. A description like "all records relating to my knee surgery, roughly spring 2024" is far more useful than a blank date range.
Can I change my mind after submitting?
Yes. Send a signed, written revocation to MISH Medical Records Dept., Attn: Release of Information, 10951 Lakeview Ave., Lenexa, KS 66219. It takes effect when we receive it, though it can't undo any disclosure already made while the authorisation was valid.
Is the form secure?
Yes. It's hosted by Updox, our health information partner, rather than on this website — which is why the form opens on a different address.
