Your health information and your rights
The law requires us to keep your medical information private, to tell you how we use it, and to follow the practices we describe. Each of the organisations below publishes its own Notice of Privacy Practices — read the one that covers where you received care.

If you're not sure which applies to you, it's the organisation named on your billing statement or appointment paperwork. When in doubt, call us and we'll tell you.

See and get a copy of your records
You can inspect your health information and request a copy, in paper or electronic form.
Ask us to correct something
If you believe your record is wrong or incomplete, you can ask us to amend it. We'll tell you in writing if we can't.
Know who we've shared it with
You can request an accounting of certain disclosures we've made of your health information.
Ask us to limit what we share
You can request restrictions on how we use or disclose your information. If you pay for a service in full yourself, you can require us not to tell your health plan.
Choose how we contact you
You can ask us to reach you at a particular address or number — a work line rather than home, for instance.
Be told if your information is breached
If there's a breach affecting your health information, we're required to notify you.
How to exercise these rights
Requesting records
Use the secure medical record request form. You'll need photo ID, and proof of authority if you're requesting on someone else's behalf.
Corrections and restrictions
Amendments, restrictions and accounting requests go to Health Information Management in writing. Call first and we'll tell you exactly what to send.
A paper copy
You're entitled to a printed copy of any of these notices, even if you've read it here. Ask at registration or call and we'll post one to you.
Making a complaint
If you believe your privacy rights have been violated, you can complain to us, to the federal government, or to both. You will not be penalised, and it will not affect your care.

Put your complaint in writing and send it to the Privacy Officer at the address below, or call and ask to be put through. We'll acknowledge it and tell you what happens next.

U.S. Department of Health and Human Services
You can file directly with the Office for Civil Rights, which enforces federal health privacy law. Complaints must generally be filed within 180 days of when you knew the problem occurred.
File with HHS Office for Civil Rights
Filing a complaint will not affect your treatment at MISH in any way.
